Introduction: Intrauterine devices (IUD) are considered to be a safe and effective method for contraception. However, they have been associated with complications such uterine perforations, hemorrhage, infection, pregnancy, pain, and translocation to adjacent organs. In such circumstances they must be removed immediately to avoid further complications.
Case Description/Methods: A 60-year old hispanic female from Cuba presented to the Gastroenterology Clinic with a two year history of left lower quadrant abdominal pain and bloating. Her past medical history was notable for hypertension, hyperlipidemia, and nonalcoholic fatty liver disease. On presentation she underwent cross sectional imaging of the abdomen and pelvis with intravenous contrast, which was notable for a 3 cm T shaped metallic foreign body in the distal colon as well as a large left adnexal mass. A therapeutic colonoscopy was performed, which revealed an IUD embedded in the wall rectosigmoid colon. A cold snare was used to successfully remove the foreign body without further complication.
Discussion: Although IUD’s are considered to be safe and effective means of contraception, they are associated with serious complications. Uterine perforation by an IUD is rare and occurs in 0.87 in 1,000 people. Patients can present with vague and unusual symptoms which may mimic other diseases. This may delay the diagnosis and cause further complications. Therefore, it is important to take a thorough history and consider such complications. Once the diagnosis is made, the foreign body should be removed immediately.
