Introduction: Colorectal cancer (CRC) is the third leading cause of cancer death worldwide. Currently, screening is suggested for persons 45-75 years with further screening based on decisions of patients and clinicians. There is limited information about the development of CRC in elderly patients beyond recommended screening age.
Methods: All patients aged 75 or older first diagnosed with CRC from January 2014 to December 2020 were identified through an on-site cancer registry at the Advocate-Aurora Healthcare System with prior history of hereditary syndromes associated with higher risk of CR. We analyzed two age groups. Group 1 had 137 patients who were from 75-84 years and Group 2 had 92 patients who were ≥85 years old. Smoking history and prior colonoscopies were examined.
Results: Of the 134 patients in Group 1, 34 had stage I CRC. 68% of these had a prior colonoscopy with 59% within the previous 5 years. Forty-seven of the patients in Group 1 had stage II CRC and 36% had a prior colonoscopy, with 20% within 5 years. Thirty-Five of the patients in Group 1 with stage III CRC with 46% had prior colonoscopies, with 24% within 5 years. Twenty-one of the patients in Group 1 had stage IV CRC and 29% had a prior colonoscopy, with 20% within 5 years of diagnosis (See Table 1). Twelve of the ninety-two patients in Group 2 had stage I CRC, 67% had previous colonoscopies with 25% within 5 years of diagnosis. And thirty-three patients in Group 2 with stage II CRC, 45% had a prior colonoscopy with 24% within 5 years of diagnosis. Twenty-seven patients in Group 2 had Stage III CRC and 44% had had a prior colonoscopy with 22% had one in the last 5 years. Twenty patients in Group 2 stage IV and 30% had colonoscopies prior with 15% within the last 5 years of diagnosis (See Table 1).
Discussion: Our study suggests that elderly patients may still benefit from ongoing screening for CRC and with fewer colonoscopies and recent colonoscopies performed in patients with more advanced CRC. Many of the elderly patients in our cohort with CRC were smokers.
