P3202 - Predicting Colon Cancer Screening: The Role of Sociodemographics and Comorbidities

Introduction: Colon cancer is the third most diagnosed cancer globally and second leading cause of cancer-related death in the United States (US). To improve survival rates, early detection is key. Sociodemographic factors and comorbidities impact screening rates. Addressing disparities requires targeted interventions such as education, improved access through insurance coverage, and routine screening for individuals with and without comorbidities.

Methods: This cross-sectional study utilized data from the 2020 National Health Interview Survey, conducted by the CDC. Data was collected through interviews, from 29,482 respondents representing 253 million US adults. Sample weights were applied to ensure generalizability, and complex sampling design was accounted for. Pearson chi-square analysis assessed group differences. A composite comorbid variable was created using COPD, obesity, and smoking status. Multiple logistic regressions identified sociodemographic factors predicting screening likelihood and evaluated the relationship between comorbidities and screening.

Results: Among the sociodemographic factors evaluated, family income-to-poverty ratio showed a significant association with screening. Those below the poverty line had lower odds of screening compared to those above it [AOR 0.71,95% CI(0.51–0.97); p=0.030]. Hispanic respondents had lower odds of screening than non-Hispanic whites [AOR 0.74, 95% CI(0.54–1.00); p=0.053] although not statistically significant. Obese respondents and smokers had higher odds of being screened [AOR 1.22, 95% CI(1.02–1.45); p=0.027] and [AOR 1.29, 95% CI(1.07–1.56); p=0.007], respectively. Having COPD did not increase the odds of screening. Respondents with two comorbidities had increased odds of screening [AOR 1.68, 95% CI(1.31–2.15); p< 0.001], and those with three had even higher odds [AOR 2.08, 95% CI(1.28 – 3.37), p=0.003]. After adjusting for sociodemographic factors and other medical conditions, the relationship remained the same in the two [AOR 1.55, 95% CI(1.19 – 2.02); p-value0.001] and three [AOR 1.90, 95% CI(1.15–3.13); p-value0.012] comorbidities groups, though lower.

Discussion: Several demographic and health factors may impact colon cancer screening rates. Lower-income individuals may face barriers to colon cancer screening. Individuals with medical comorbidities may be more likely to seek medical care and receive screening due to their increased healthcare needs. Healthcare providers need to be aware of these factors and work to address disparities
