Introduction: To study the protection offered by current PPI use against various UGI pathologies and subsequent bleeding.
Methods: Retrospective chart review study of all patients presenting with UGIB to an inner-city hospital over a five-year period was conducted. Endoscopy findings with or without active bleeding were recorded. A descriptive and chi square testing analysis was used to obtain any significant protective role offered by the current use of PPI.
Results: Two hundred and twenty patients were identified during the five-year period presenting with UGIB. One hundred and forty-two were male (64.5%) and seventy-eight females (34.5%), age range was 19-93. Of these patients, 21.4% were on PPI treatment and 78.6% were not. Current PPI use was found to be protective against the severity of gastric pathology (single v/s multiple gastric ulcers, gastritis Vs not, erosive gastritis Vs not) but not esophageal or duodenal pathology, or against bleeding due to any pathology. The protection of PPI use is shown in Table 1.
Discussion: Endoscopy performed in patients presenting with UGIB showed less severe gastric pathology but did not translate to protection against esophageal or duodenal pathology in patients on current PPI.
Current PPI use did not offer protection against GI bleeding from various pathologies.
PPI use did not affect the HP detection rates.
