P3586 - Outcomes of Inflammatory Bowel Disease (IBD) Associated Hospitalizations with Eating Disorders in the United States Between 2009 and 2018 Using the National Inpatient Sample

Introduction: Although eating disorders including anorexia nervosa, bulimia and psychogenic vomiting often share symptoms with inflammatory bowel disease (IBD), there is a scarcity of data describing outcomes of hospitalized IBD patients with eating disorders. This study describes the outcomes of IBD associated hospitalizations with eating disorders compared to IBD patients without eating disorders in the United States between 2009 and 2018.

Methods: Using the National Inpatient Sample (NIS), all admissions with primary or secondary discharge diagnosis of Inflammatory Bowel Disease between the years 2009 and 2018 were evaluated. IBD patients with eating disorders was defined as IBD in patients who had anorexia nervosa, bulimia, avoidant restrictive food intake disorder, and binge eating. ICD codes were utilized to identify IBD and patient related comorbidities. Subsequently, we analyzed the differences between the eating disorder IBD hospitalized cohort and control cohort in terms of demographics, patient characteristics, and impact on patient care. Multivariate logistic regression analysis was performed to adjust for confounders. All statistical analysis was performed using STATA software.

Results: The NIS cohort yielded a total of 876 IBD patients with eating disorders in a sample of 1.7 million hospitalized IBD patients between the years 2009 and 2018. Hospitalized IBD patients with eating disorders were more likely to be admitted with Ulcerative Colitis than Crohn’s disease. IBD related complications such as malnutrition and anemia were significantly more common in the IBD patients with eating disorders. There were no statistically significant differences in IBD related comorbidities such as pneumonia, sepsis, VTE and C. difficile infection between the cohorts. When adjusted for confounders, eating disorder was an independent predictor of in-hospital mortality in hospitalized IBD patients (OR 10.83 (95% CI), p < 0.006).

Discussion: Our study demonstrates that eating disorders in hospitalized IBD patients is an independent risk factor for mortality. Hospitalizations for IBD patients with eating disorders were more likely to be complicated by malnutrition and anemia. These findings highlight the need for identifying eating disorders in the hospitalized IBD population and the potential importance of multi-disciplinary care involving psychiatrists and nutritionists to prevent complications and increased risk of mortality in this patient population.
