Introduction: Colonoscopy is the gold standard screening method that has shown to decrease mortality from colorectal cancer (CRC). The G-EYE is an an add-on device targeted to increase efficacy of CRC screening. G-EYE comprises of a balloon, inflated on withdrawal to assist with straightening colon folds and stabilizing the optics. In this meta-analysis we attempted to investigate the efficacy of this device in improving ADR and polyp detection rate (PDR).
Methods: The literature search through databases of Embase (Embase.com) and Medline (PubMed) was performed through April 20th, 2023. All studies addressing the following were selected: (1) Patients: undergoing colonoscopy for screening/surveillance and/or diagnostic reason. (2) Outcomes: ADR and PDR (3) Comparison/intervention: High-definition colonoscopy (HD), balloon/retrograde endoscopes (G-EYE). Study screening and data extraction was performed by two independent reviewers (H.H. and M.A.). Statistical analysis of pooled rates of each outcome was performed with OpenMeta[Analyst] (CEBM, Brown University, Providence, Rhode Island, USA). Odds ratio (OR) along with 95% confidence interval (CI) was used with a p-value of < 0.05 considered statistically significant.
Results: The search strategy finalized 4 studies with 3980 total colonoscopies after implementing inclusion and exclusion criteria (table 1). The mean age ranged from 63.4 to 65.4 years old (table 1). Improved pooled ADR and PDR with G-EYE was observed throughout studies (Odds ratio [OR]= 1.744, CI: 1.534 – 1.984, P< 0.001, I2= 0%) and (OR 1.766, CI: 1.547 – 2.015, P< 0.001 I2= 0%), respectively (table 1, figure 1).
Discussion: Our meta-analysis suggests that G-EYE can improve both ADR and PDR. Larger RCTs with longer periods of follow-up measuring more variables are required to investigate whether G-EYE can improve the interval CRC and reduce mortality.
