Introduction: Colonoscopy is the gold standard for colorectal cancer screening, offering diagnostic and therapeutic intervention. Colonoscopy bowel preparation directly impacts the quality of colonoscopy and adenoma/polyp detection. About 10-25% of bowel preparation are inadequate due to patient-related and procedure-related factors. We performed a single-center study to assess the knowledge of clear liquid diet (CLD) among individuals undergoing outpatient diagnostic and screening colonoscopies.
Methods: Adult patients aged 18 to 80 undergoing outpatient colonoscopies were voluntarily surveyed at the endoscopy unit between November 2021 to March 2023 before their colonoscopy. The survey included questions addressing demographics (Age, Sex, Education level), comorbidities, source of bowel preparation instructions, and identification of CLD. Under the CLD section, patients' understanding of CLD for bowel preparation is assessed based on their ability to correctly identify items belonging to a CLD, among other food choices. Exclusion criteria were inpatient colonoscopy and incomplete surveys. Descriptive statistics were shown as numbers and percentages. Chi-square and Fisher exact tests were performed to test the association for categorical variables.
Results: A total of 430 patients filled out the survey, and 400 were eligible for the study with a mean age of 53. As shown in Table 1, participants were predominantly female 66.5% vs male 35.5%. Overall, 225 patients self-identified as Black (56.3%), 161(40.3%) as White, and 13 (3.3%) as Other (Asian, Hispanic, American Indian). 194 (48.5%) achieved a high school education, 85 (21.3%) college level, 47 (11.8%) university level, and 60 (15%) vocational/other. The patient's source of bowel preparation is outlined in Table 1.
Table 1 shows that a higher proportion of all three demographics knows what CLD is (97%), with a significant association with gender (P= 0.0035) and education (P= 0.0328).
In contrast, the identification of various CLDs was lower in all demographics (Figure 1). Plain chicken broth was the most correctly identified CLD (67-85%) (Figure 1).
Discussion: After the analysis, we found that having a higher level of education gave the patients the impression of understanding what constitutes CLD. However, the percentage of patients who correctly identified CLD was very low across every educational level. This pattern was also observed when our data were stratified by gender and race.
