P0017 - Chronic Disease and Lifestyle Factors Associated With Chronic Pancreatitis: A Nationwide Database Study

Introduction: Chronic Pancreatitis (CP) involves progressive fibrosis of the pancreatic parenchyma in response to inflammation. The progression of CP is influenced by a combination of genetic and environmental factors. It is reported that chronic disease can affect pancreatic function. For instance, diabetic exocrine pancreatopathy leads to subclinical pancreatic fibrosis in the absence of CP. Additionally, patients with chronic kidney disease (CKD) are reported to have abnormal exocrine pancreatic function. However, an association between CP and other common chronic diseases and lifestyle factors have not been studied. The aim of this study is to identify associations of chronic diseases and lifestyle factors in a population of CP patients.

Methods: Using the Nationwide Inpatient Sample 2016-2018 database, we identified patients with CP utilizing ICD 10 billing codes. This cohort was matched in a 1:1 ratio with a cohort of patients without CP via propensity score for age, gender, race, alcohol use, cholelithiasis, acute pancreatitis, obesity, hyperlipidemia, and coronary artery disease. Post-match multivariate logistic regression was performed to assess lifestyle and chronic disease risk factors for CP. Post-match modelling showed p >0.05 for all matched variables and variance ratios were all > 2 indicating the data was appropriately balanced.

Results: A total of 109,648 patients with CP, and a matching cohort of 109,648 patients without CP were studied. Mean age was 52.5 (±15.4), and 56% were male in the CP group. When compared to those without CP, patients with CP had statistically significant higher odds for Type 1 diabetes mellitus (DM1), type 2 diabetes mellitus (DM2), CKD, end-stage renal disease (ESRD), hypertension, cannabis use and tobacco use. Additionally, patients with CP had significantly lower odds of aspirin use. The prevalence of metabolic syndrome was not different between the 2 groups. The absence of all analyzed risk factors showed a significantly decreased association with CP (OR 0.75; 95% CI 0.74 – 0.76; < 0.001).

Discussion: Patients with CP have a higher prevalence of chronic diseases such as DM, CKD, ESRD and hypertension. Additionally, modifiable lifestyle risk factors such as cannabis use and tobacco use also have an association with CP. The management of chronic disease and lifestyle modifications may play a significant role in the treatment of CP. Future studies will have to look at these individual risk factors to delineate more information on their associations.
