P0024 - Prior Statin Use on Outcomes in Acute Pancreatitis: A Propensity Score Analysis

Introduction: Statin medications are lipid-lowering agents that have a number of pleiotropic properties including anti-inflammatory and immunomodulatory effects. Statins have been hypothesized to exert a protective role in acute pancreatitis, but their impact on the outcomes of pancreatitis remains unclear. We aim to evaluate the effect of prior statin use on outcomes in acute pancreatitis using a propensity matched cohort.

Methods: We evaluated a prospective cohort of patients admitted with acute pancreatitis to Los Angeles General Hospital from February 2015 to October 2022. Statin use prior to admission was determined by reviewing the outpatient medication reconciliation and documentation. A propensity model was constructed using variables that may influence statin intake including patient demographics, clinical characteristics, and medical comorbidities. Patients taking statins were matched 1:1 to patients not taking statins using propensity scores. Logistic regression analysis was performed using the propensity matched cohort. Outcomes included severity of pancreatitis, presence of local complications, development of severe inflammatory response syndrome (SIRS), organ failure, intensive care unit (ICU) admission, and mortality.

Results: A total of 1257 unique patients presenting with acute pancreatitis were evaluated. 138 patients (11.0%) were taking a statin medication prior to admission. After propensity score matching, 123 statin users were matched with 123 non-statin users. Baseline demographics and characteristics were similar between groups (Table 1). On logistic regression, statin users were less likely to present with SIRS than non-statin users (OR 0.39, 95% CI 0.22-0.72) with a trend towards reduced mortality (OR 0.19, 95% CI 0.02-1.68). There was no significant difference in the development of moderate or severe pancreatitis (OR 0.96, 95% CI 0.53-1.73), local complications (OR 1.18, 95% CI 0.38-3.61), organ failure (OR 1.23, 95% CI 0.65-2.32), ICU admission (OR 0.82, 95% CI 0.44-1.52), or mortality (OR 0.19, 95% CI 0.02-1.68).

Discussion: Patients taking statin medications admitted with acute pancreatitis are less likely to present with SIRS compared to non-statin users. This study highlights the potential protective role of statin use in acute pancreatitis. Randomized controlled trials are needed to evaluate the role of statins as a therapeutic agent in the management of acute pancreatitis.
