P0603 - Does Race Affect Gastrointestinal Bleeding Outcome in Atrial Fibrillation Patients on Oral Anticoagulation Therapy: Nationwide Inpatient Sample Study

Introduction: Patients with atrial fibrillation often started on anti-coagulation to prevent the risk of stroke. Gastrointestinal bleeding (GIB) is the major site of bleeding associated with anticoagulation and it was associated with significant mortality morbidity rates among these patients. In the US guidelines, the race was not specified as a factor associated with the risk of bleeding. The primary aim of our study was to assess the effect of race on gastrointestinal bleeding among atrial fibrillation patients using anticoagulants.

Methods: A retrospective analysis was performed on patients admitted to the hospital with GIB and concomitant diagnosis of atrial fibrillation, using the Nationwide Inpatient Sample (NIS) database between 2016-2018. Subsequently, these patients were classified based on their race into Black and non-Black populations. Patient demographic factors, Elixhauser comorbidity index, bleeding severity indicators, and hospital characteristics were used to calculate propensity scores. The average treatment effect on the treated weight (ATT) was calculated based on the propensity scores and adjusted for confounders.




Results: A total of 307,045 patients were admitted with GIB and had the diagnosis of atrial fibrillation. 30315 patients were Black, 15820 (52.2%) of them were females. There was no difference in mortality among the Black population compared to the non-Black population, 0.93 odds ratio (95% CI 0.77 - 1.12, P 0.468). The adjusted mean difference in length of stay in the hospital increased in the Black group (95% CI 0.46 0.28 - 0.64, P < 0.001). The odds ratio of hospitalization charges, discharge to rehab, and transfer to another hospital were insignificant in the Black group. (Table 1).

Discussion: We found that Black patients are younger but had a higher burden of comorbidities compared to other ethnicities. Also, it was found that Black patients have a higher rate of being discharged with a nurse compared to the non-Black population as well as a significantly longer hospital stay.
