Introduction: Cirrhosis is an increasingly common condition that can emerge as the terminal stage of many liver diseases. New Mexico has the highest liver disease mortality at 41.1 deaths per 100 000 residents (CDC, 2022). Decompensated cirrhosis has a high symptom burden from its multiple complications leading to high healthcare utilization. This results in repeat readmissions, which can strain healthcare systems at capacity. Readmission rates at 90 days are an important outcome measure in quality improvement efforts. Studying patient-specific and hospitalization-specific characteristics associated with 90-day readmission can help decrease overall hospital burden and improve care.

Methods: A retrospective, case-control study was performed to identify patients admitted with cirrhosis-related complications between 7/1/2019 and 1/1/2020 at a safety-net academic medical center. Data was collected regarding demographics, in-hospital events, and discharge/readmission. Patients with readmission within 90 days were identified. Patients without readmission were compiled into a control group. Multivariate logistic regression on Prism 9 (GraphPad) was used to identify statistically significant factors for readmission (P< 0.05).

Results: 442 patients were identified for this study. 95 (21.5%) were readmitted within 90 days. 347 (78.5%) were used as a control group. Factors significantly associated with readmission include nosocomial infection (aOR 5.97, 95% CI 1.35-28.47), proton pump inhibitor (PPI) prescribed on discharge (aOR 2.06, 95% CI 1.14-3.75), discharge against medical advice (aOR 3.12, 95% CI 1.06-9.00), and discharge to long-term acute care (aOR 2.77, 95% CI 1.04-7.19). Nosocomial infections in readmitted patients had an intra-pulmonary (50%) or intra-abdominal source (50%).

Discussion: This study shows that patients admitted with a cirrhosis-related diagnosis were more likely to be readmitted if they had a PPI prescription on discharge, left against medical advice, had a nosocomial infection, or discharged to long-term acute care. Long-term PPI use without an evidence-based indication is linked to SBP, pneumonia, hepatic encephalopathy, and mortality. PPIs may alter the intestinal microbiome, resulting in bacterial translocation. PPI stewardship is a potential quality improvement intervention that may reduce hospital readmission.
