P0916 - Alcoholic Cirrhosis Associated With a Higher Incidence of Spontaneous Bacterial Peritonitis

Introduction: Spontaneous bacterial peritonitis (SBP) is an ascitic fluid infection common in patients with cirrhosis leading to a high rate of morbidity and mortality. This study is centered at a northeast liver transplant center. Our aim is to analyze multiple etiologies of cirrhosis and determine which type of cirrhosis is associated with a higher incidence of SBP.

Methods: In a retrospective cohort study, ICD codes-10 were used to identify patients diagnosed with Cirrhosis and SBP. Inclusion criteria comprised patients admitted to Einstein Medical Center Philadelphia and Montgomery Campus, PA from January 2017 to January 2022. Patients under the age of 18, pregnant, or with a history of liver transplant were excluded. Baseline characteristics were presented as frequency (percent) for categorical variables and as means and interquartile range (IQR) for continuous variables. Mann–Whitney U and χ2 tests were performed to evaluate differences between groups. Age, gender, race, and etiology of cirrhosis were collected as independent variables to analyze the association with the following dependent variables: Presence vs. absence of SBP in patients with cirrhosis.

Results: Of the 3999 patients with cirrhosis, the mean age was 58.8±10.9. 2414 (60%) patients were males and 1585 (40%) were females. No significant associations were found between genders when compared to SBP vs. non-SBP group, p < 0.651. 1295 (32.4%) had alcoholic cirrhosis, 1248 (31%) had Hepatitis C cirrhosis, 137 (3.4%) had NASH cirrhosis, 1147 (28.7%) were categorized as unknown, and 172 (4%) as Other, which included autoimmune, PBC cirrhosis, cardiac, HBV and Sarcoidosis. Patients with alcoholic cirrhosis had the highest incidence of admissions with SBP, 8% (n = 101) when compared to patients with non-alcoholic cirrhosis, p < 0.001.

Discussion: Amongst patients with cirrhosis hospitalized in the 5-year timeframe, patients with alcoholic cirrhosis were found to have a significantly higher incidence of SBP compared to other etiologies of cirrhosis. As SBP continues to carry a high rate of mortality, raising the index of suspicion for a specific cohort of cirrhosis patients can accelerate the time and quality of care provided. Further investigation is needed to determine its impact on morbidity and mortality.
