P1345 - Temporal Trends in the Risk of New-Onset Gastroparesis Following SARS-CoV-2 Infection: A Propensity Matched Cohort Study

Introduction: SARS-CoV-2 infection is known to present with GI manifestations, which may precede respiratory symptoms. These manifestations typically include loss of appetite, diarrhea, nausea, vomiting, and abdominal pain. While there are case reports in the literature describing gastroparesis after COVID-19, large-scale studies are currently lacking. To address this gap, we aim to investigate the risk of gastroparesis after COVID-19 using a large database.

Methods: We utilized the TriNetX multi-institutional database to examine the risk of new diagnosis of gastroparesis (ICD-10 code K31.84) in adult patients (age 18-65) with COVID-19 between April 2020 and December 2021. We compared this group to patients who had other viral respiratory infections (VRI) but did not have COVID-19. Patients with a history of bariatric surgery or gastroparesis, prior to the index event, were excluded from the study. To ensure balanced cohorts, we performed 1:1 propensity score matching for age, sex, race, ethnicity, diabetes mellitus, opioid use, fundoplication, and hospitalizations.

Results: During the study period, there were 5,566,643 patients diagnosed with COVID-19 (57 % females and 61% white) and 499,709 patients with other VRI (64% female and 65% white). After propensity score matching, both the COVID-19 and other VRI cohorts consisted of 499,718 patients. Patients with COVID-19 had a higher risk of newly diagnosed gastroparesis compared to patients with other VRI at multiple time intervals from index event. Specifically, at 1 month (odds ratio (OR) 4.00, 95% confidence intervals (CI) 2.77-5.76, P < 0.0001), 3 months (OR 2.66, 95% CI 2.11-3.36, P < 0.0001), 6 months (OR 2.26, 95% CI 1.89-2.71, P < 0.0001), and 12 months (OR 1.90, 95% CI 1.67-2.18, P < 0.0001) after COVID-19 infection.

Discussion: Our study results suggest that patients with COVID-19 have a higher risk of developing new onset gastroparesis compared to those with other VRI. While viral infections have previously been linked to the onset of gastroparesis, our study reveals an even higher risk among individuals with COVID-19, raising concerns regarding potential long-term complications following the infection. Interestingly, the increased risk seems to diminish with passage of time. Therefore, long-term monitoring and management of gastrointestinal symptoms may be necessary in COVID-19 patients. Further studies are needed to confirm these findings and investigate the underlying mechanisms.
