P0688 - The Impact of Clinical Remission on Healthcare Resource Utilization Among Patients with Moderate to Severe Crohn´s Disease and Ulcerative Colitis

Introduction: Optimal management of Crohn’s disease (CD) and ulcerative colitis (UC) aims to induce and maintain clinical remission which may subsequently lower healthcare resource use (HCRU). This study assessed the potential impact of achieving CD or UC remission on HCRU among patients with moderately to severely active CD or UC.

Methods: Data were drawn from an Adelphi Real World Inflammatory Bowel Disease (IBD) Disease Specific Programme^TM, a cross-sectional survey collected from gastroenterologists (GI) and their IBD patients (>18 years, GI-confirmed diagnosis and not involved in a clinical trial) globally (France, Germany, Italy, Spain, United Kingdom, and the United States) from September 2020 to January 2021. GIs completed a record form for their next 5-10 consulting IBD patients covering demographics, remission status, healthcare visits, and past CD/UC-related hospitalizations and surgeries. Patients with moderately to severely active CD/UC (subjectively classified by their GIs at initiation of current treatment) were stratified based on GI-stated current clinical remission status (i.e., remission [R] vs non-remission [NR]). HCRU outcomes from the past 12 months were compared between patients in R vs NR using inverse probability weighted regression statistical analysis (α=0.05), adjusting for age, sex, body mass index, smoking status, disease location, and treatment.

Results: A total of 1,720 CD patients were analysed (R: n=1,095; NR: n=625; Figure 1), mean age 38.8 years, 52.6% male, and mean disease duration 5.6 years. Compared with CD patients in NR, patients in R experienced a lower risk of at least one hospitalization (12.8% vs 29.7%; p< 0.001) and surgery (13.0% vs 24.0%; p< 0.001; Figure 2); visits to healthcare professionals in the last 12 months were not significantly different between R and NR groups (6.3 vs 6.7; p=0.155). In addition, 1,748 UC patients were analysed (R: n=1,124; NR: n=624; Figure 1), mean age of 40.4 years, 53.9% male, and mean disease duration 4.9 years. Compared to UC patients in NR, patients in R experienced a lower risk of at least one hospitalization (10.9% vs 21.7%; p< 0.001) and surgery (1.8% vs 4.0%; p=0.017; Figure 2), and fewer healthcare professional visits (5.7 vs 6.3; p=0.028).

Discussion: In a global sample of patients with IBD, those in clinical remission had lower risk of hospitalization and surgery in the last 12 months and may also experience lower HCRU burden.
