Introduction: Inflammatory Bowel Disease (IBD) is an important cause of morbidity and mortality worldwide. Due to variable demographics among the US states, we aim to determine trends in the prevalence, incidence, mortality, and disability-adjusted life years (DALYs) in the United States and across different states.

Methods: Using the Global Burden of Disease database, we analyzed the prevalence, incidence, mortality, and DALYs in the United States between 1990 and 2019. We computed relative percentage changes (RPC) over the study period and compared these between states.

Results: The overall age-standardized prevalence of IBD in the US has increased minimally from 192.4 per 100,000 population in 2000 to 193 per 100,000 population in 2019 (age-standardized RPC +0.3%). Illinois, Alabama, and North Carolina had the greatest increase in prevalence (+4.3%, +3.3%, and +2.9%) among all states. However, the District of Colombia, North Dakota, and California had the greatest decrease in prevalence (-3.5%, -3.3%, and -2.2%). The incidence of IBD increased in the US over the 30 years from 21.3 per 100,000 population in 2000 to 23.2 per 100,000 population in 2019 (RPC=+9.4%). North Dakota, South Dakota, and Ohio had the highest increase in incidence (+23.4%, +22.4%, and +22.2%). All states had an increase in the incidence of IBD except Nevada, where incidence rates decreased by -1.8% during the study period. The mortality rates increased in the US from 0.84 per 100,000 population in 2000 to 1.02 per 100,000 population in 2019 (RPC +21.4%). Tennessee, Iowa, and Kentucky had the highest increase in mortality (+39.6%, +39.1%, and +38.3%). Mortality rates increased in all states except Nevada (-2.5%). Women had a higher increase in IBD mortality than men (+24.1% vs. +21.7%). In the US, age-standardized DALYs increased for both genders, with a larger decrease for women than men (+8.1% vs. +6.1%).

Discussion: Overall, IBD incidence, prevalence, and mortality have increased significantly in the US since 2000. Urgent attention is necessary to address these changes and the geographical, socioeconomic, and sex disparities in mortality from IBD.
