Introduction: Inflammatory Bowel Disease (IBD) is an important cause of morbidity and mortality worldwide. Due to different demographics among the different countries of Middle East and North Africa (MENA) region, we aim to determine trends in the prevalence, incidence, mortality, and disability-adjusted life years (DALYs) in the MENA region and across different countries and genders.
Methods: Using the Global Burden of Disease database, we analyzed the prevalence, incidence, mortality, and DALYs in the MENA region between 1990 and 2019. We computed relative percentage changes (RPC) over the study period and compared these between MENA countries.
Results: The age-standardized prevalence rate of IBD in the MENA region has increased from 36.2 per 100,000 population in 1990 to 48.2 per 100,000 population in 2019 (age-standardized RPC=33%). Turkey, Jordan, and Iraq had the greatest increase in prevalence (+95%, +64%, and +34%) among all countries. The prevalence increased in all countries except Kuwait, which had a decrease in prevalence over the study period (-2%). The incidence of IBD increased in the MENA region over the 30 years from 2.95 per 100,000 population in 1990 to 3.74 per 100,000 population in 2019 (RPC=+27%). Turkey, Jordan, and Lebanon had the greatest increase in incidence (+67%, +57%, and +52%). The increase in incidence rate for IBD was higher in males (+28%) compared to females (+26%). The mortality rates decreased in the MENA region from 0.34 per 100,000 population in 1990 to 0.26 per 100,000 population in 2019 (RPC -24%). Jordan, Turkey, and Palestine had the greatest decrease in mortality over the study period (-46%, -40%, and -40%). The decreased mortality was found in all countries except Yemen, Sudan, United Arab Emirates and Libya (+19%, +19%, +13%, +1%). In the US, age-standardized DALYs decreased for both genders, with a larger decrease for males than females (-9% vs. -6%) for the past 30 years.
Discussion: IBD incidence, prevalence, and mortality have changed significantly in the MENA region since 1990. Urgent attention is necessary to address these changes and the geographical, socioeconomic, and sex disparities in incidence, prevalence, and mortality from IBD.
