Introduction: Inflammatory bowel disease (IBD) can have manifestations in the upper gastrointestinal tract. Lymphocytic esophagitis, focally enhanced gastritis, and duodenitis have been found in pediatric IBD. However, it is unclear if gastroparesis and functional dyspepsia are associated with IBD. The aim of the study was to determine the prevalence of common upper gastrointestinal disorders in Crohn’s disease (CD) and ulcerative colitis (UC).
Methods: A population-based cohort analysis was performed using TriNetX. Patients with CD and UC were identified, respectively. Patients need to have a colonoscopy within one year of diagnosis of CD or UC to ensure diagnostic accuracy. Propensity score matching was performed to account for the differences of age at diagnosis, gender, and race. We compared the risks of gastroparesis, functional dyspepsia, gastritis, duodenitis, Gastro-esophageal reflux disease (GERD), erosive esophagitis and Barrett’s esophagus in CD and UC. Odds ratio (OR) with 95% confidence interval was used to compare the cohorts.
Results: We identified a total of 69,415 patients with CD (mean age 40.7 years old, female 53.5%) and 77,021 patients with UC (mean age 47.4 years old, female 51.7%). The risks of developing GERD (OR 1.26-1.36), erosive esophagitis (OR 1.14-1.38), functional dyspepsia (OR 1.11-1.32), gastroparesis (OR 1.19-1.54), gastritis (OR 1.16-1.31), duodenitis (OR 1.37-1.61) were higher in patients with CD than UC (all p< 0.0001). The risks of Barrett’s esophagus were similar in both CD and UC (p=0.07).
Discussion: Patients with CD were at higher risk for developing upper gastrointestinal disorders than UC, including GERD, erosive esophagitis, functional dyspepsia, gastroparesis, gastritis, and duodenitis.
