Introduction: Prognostic scores are very helpful in predicting patient survival in chronic liver disease (CLD). Recently, the albumin-bilirubin (ALBI) score has been found to be useful as a prognostic indicator in liver related diseases. However, there is paucity of data on its usefulness in alcohol associated liver disease (ALD). Therefore, we aimed to assess the prognostic accuracy of ALBI score and compare it with MELD, MELD-Na+ and CTP scores in predicting survival in patients with ALD.
Methods: This study was conducted on consecutive ALD patients either attending the outpatient clinic or hospitalized between November 2019 and November 2022 On hospitalization, demographic, clinical and laboratory parameters were recorded and the ALBI score, MELD, MELD-Na+ and CTP scores were calculated. Subsequently, the accuracy in predicting survival up to 3 years was compared between the above scores.
Results: 490 ALD patients were included in the study. Higher ALBI score was seen in ALD patients who died during hospitalization (0.464 versus 0.211; P < 0.001), at 28 days (0.379 versus 0.137; P < 0.001), 90 days (0.331 versus 0.063; P < 0.001), 6 months (0.322 versus 0.017; P < 0.001), 1 year (0.316 versus -0.003; P < 0.001), 2 years (0.294 versus -0.041; P < 0.001), and 3 years (0.287 versus-0.037; P < 0.001) in comparison to patients who survived. However, the AUROC values of ALBI were inferior to MELD, MELD-Na+ and CTP scores in predicting survival up to 3 years (Table - 1). Further, on Cox regression analysis, parameters used for estimation of MELD, MELD-Na+, CTP (serum creatinine, serum sodium, and encephalopathy; p< 0.001) were found to be independent predictors of mortality in contrast to the parameters serum albumin and serum bilirubin used for estimation of ALBI score.
Discussion: All admitted ALD patients had grade 3 ALBI score and the non-survivors had significantly higher ALBI score in comparisons to survivors. MELD, MELD-Na+ and CTP scores were better predictors of survival during hospitalization and also up to 3 years compared to the ALBI score, which was found to be inferior to the other conventional prognostic scoring systems.
