Introduction: We present a 29-year-old female with metastatic neuroendocrine carcinoma. Around the time the malignancy was discovered, she was found to be pregnant. She delivered a premature infant at 25 weeks. Unfortunately, her course precipitously declined following giving birth. She soon after developed fever and encephalopathy. Patient was admitted to the MICU with maximal support and aggressive treatment. Goals of care discussion with family and patient ultimately resulted in comfort focused treatment with patient being placed on hospice. She ultimately succumbed to her illness. This case will highlight the hospital course of a 29-year-old pregnant female with metastatic neuroendocrine carcinoma.
Case Description/Methods: Patient is a 29-year-old female with history of metastatic neuroendocrine carcinoma, ulcerative colitis, right portal vein thrombosis (on rivaroxaban), recent pregnancy (s/p Cesarean delivery), hepatic abscess complicated by bacteremia (s/p drainage with drain present, treated with doxycycline) who presented to the ED with low back pain after a fall. This case highlights her ICU course including her clinical course with the illness.
Discussion: Neuroendocrine tumors (NETs) are rare, accounting for less than one percent of all malignant disorders in the United States. It is estimated that fewer than 2,000 new cases occur in the United States each year. Neuroendocrine tumors (NETs) are a heterogeneous group of neoplasms that arise from cells throughout the neuroendocrine system, most commonly arising from the gastrointestinal (GI) tract, lungs, and bronchi. They are classified into several broad groups including carcinoid tumors (encompassing NETs of the GI tract, lung, and thymus), NETs of the pancreas, NETs of unknown primary, adrenal gland tumors, pheochromocytoma/paraganglioma, and extrapulmonary poorly differentiated neuroendocrine carcinoma. Further, these can be subclassified based on location and functional status. This case will highlight the hospital course of a 29-year-old pregnant female with rare metastatic neuroendocrine carcinoma.
